Improving health outcomes in south eastern Melbourne
South Eastern Melbourne Primary Health Network (SEMPHN) is a trusted partner in effectively facilitating improved health outcomes for people in South Eastern Melbourne.
As a leader in primary health care design and delivery, SEMPHN commissions out-of-hospital services locally. We make quality care more accessible and integrated, and easier to navigate – especially for people who are vulnerable and/or marginalised.
Primary care refers to those services in the community that people go to first for health care: general practices, Aboriginal Community Controlled Health Services (ACCHS), community pharmacies, many allied health services, mental health services, community health and community nursing services and dental services.
SEMPHN’s catchment covers a total geographical area of 2,935 square kilometres and serves a highly diverse population of just over 1.6 million people: this is almost one quarter of all Victorians

Our Growth
According to estimated resident populations between 2018 and 2036, largest amounts of growth within south east Melbourne are expected in the City of Casey – adding around 182,000 people, with the City of Cardinia predicted to experience the fastest rate of growth (average 3 per cent growth rate).
First Nations People
The traditional custodians of the land our catchment covers are the Boon Wurrung and Wurundjeri peoples.
Over 7,200 people in our region identified as Aboriginal and/or Torres Strait Islander in the 2016 Census.
The highest proportions of First Nations People live in LGAs City of Frankston (1.2%), Mornington Peninsula Shire (1%) and Cardinia Shire (1%); higher than the overall Greater Melbourne proportion of 0.6%.
Commissioning
In 2020-21, SEMPHN invested just over $53 Million to commission new and existing services to help improve and increase access to health services across the catchment.
We commission services and work in partnership with local stakeholders to help people in south east Melbourne get the health care they need, when and where they need it.
